EACH registered guest over the age of 18 MUST sign a separate form.

STATEMENTS OF CONSENT & WAIVER OF LIABILITY

ALL registered guests, over the age of 18, are required to sign a Statement of Consent & Waiver of Liability form acknowledging they have read, understand and agree to the statements listed.  These statements also apply to all the minors listed on this registration.  
Please read and initial each item below, then provide the requested information and signature at the bottom of the form.
I have read and understand the Fundamental Belief Statement stated by Corner Fringe Ministries (Church), which can be found at cornerfringe.com/we-believe. By registering for this event, I agree to keep all Midrash and conversations in love and a peaceable spirit without creating discord or contention among the community.
In addition to following local laws and statutes, I also agree to abide by the rules and regulations set forth by Corner Fringe Ministries.
I release from liability and waive my right to sue the Church, their employees, officers, volunteers and agents from all claims, including, but not limited to, claims of negligence, any physical injury, illness (including death) or economic loss I may suffer as a result in my participation in this event or activities at this event.
If I need medical treatment as a result of my participation in this event, including travel to and/or from a medical facility, I agree to be financially responsible for any costs incurred as a result of such treatment.  I am aware that the Church does not provide health insurance for me and that I am responsible for my own health insurance.
I agree to hold the Church harmless from all claims, loss of damage to my personal property, liabilities and costs, including attorney’s fees, as a result of my participation in this event.  If the Church incurs any of these types of expenses due to my, or my family’s actions and/or participation, I agree to reimburse the Church.
I state that I am the parent or legal guardian of the listed minor participant(s) in this registration.  I understand that I am responsible for the obligations and acts of the minors attending this event in my care.  I understand I am physically and/or financially responsible for damage that may occur from their actions.
I understand that the Church and others may take photos/videos of registered minors or myself on this registration that may be used for future promotional items, social media posts, or informational packets.
I have read this document, and I am signing it freely.  I understand the legal consequences of signing this document and assume all risks of participating in this event.
